We are seeking a compassionate and experienced Social Worker to support patients and their families by addressing social, emotional, financial, and practical challenges that may affect their health and overall wellbeing.
The Social Worker will collaborate closely with physicians, nurses, case managers, psychologists, and other members of the multidisciplinary healthcare team to ensure patients receive appropriate support, resources, and continuity of care.
Key Responsibilities
Assess patients’ social, emotional, family, and psychosocial needs.
Provide counseling, emotional support, and crisis intervention when required.
Support patients and families in coping with illness, hospitalization, disability, or major life changes.
Develop appropriate social care plans based on individual patient needs.
Coordinate with healthcare professionals to support comprehensive patient care.
Connect patients and families with community resources, financial assistance, social services, rehabilitation programs, and other support services.
Participate in discharge planning and help ensure safe transitions from hospital to home or other care settings.
Advocate for patients’ rights, needs, and access to appropriate services.
Provide guidance to families regarding available support options and care arrangements.
Maintain accurate and confidential case notes, assessments, and documentation.
Identify safeguarding concerns and follow appropriate reporting and escalation procedures.
Ensure compliance with hospital policies, professional standards, and confidentiality requirements.
Requirements
Bachelor’s degree in Social Work or a closely related field from a recognized university.
Previous experience as a Social Worker, preferably within a hospital, healthcare, rehabilitation, mental health, or community-care environment.
Strong understanding of psychosocial assessment, patient support, and case coordination.
Strong empathy, active listening, and counseling abilities.
Ability to handle sensitive and confidential situations professionally.
Ability to work effectively within a multidisciplinary healthcare team.
Excellent spoken and written English.
